As a Home Care Assistant, you will play a crucial role in delivering essential support to participants within their residences, while also actively engaging in the collaborative care planning process as a valued member of our Interdisciplinary Team (IDT).

We are dedicated to providing exceptional care for our participants and fostering a positive work atmosphere for our staff.

Key Responsibilities:

The Home Care Assistant will be responsible for delivering personal care and assisting with daily living activities (ADLs) for participants in their homes. This includes, but is not limited to:

  • Assisting participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Measuring and documenting vital signs as directed.
  • Helping with light meal preparation and ensuring participants receive adequate nutrition and hydration.
  • Facilitating safe transfers in and out of the home and providing medical escort services as required.
  • Supporting the Rehabilitation department by implementing home exercise programs as outlined in the care plan, following appropriate training.

Additionally, you may provide translation services when qualified, ensuring effective communication and understanding.
