About the Role:

  • Provide skilled physical therapy services to patients in their homes, promoting independence, mobility, and overall well-being.
  • Develop and implement individualized treatment plans tailored to each patient's unique needs and goals.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Maintain accurate records and documentation, adhering to industry standards and regulations.
  • Pursue ongoing education and professional development to stay current with best practices and advancements in the field.

Requirements and Qualifications:

  • Bachelor's Degree or equivalent from an accredited physical therapy program (Required)
  • Master's Degree (Preferred)
  • 6 months of relevant experience Preferred
  • Current licensure as a Physical Therapist in the state of Michigan (Required)
  • Basic Life Support (BLS) certification within 90 Days of Hire (Required)
