We are seeking a compassionate and skilled LPN to join our Palliative Care team at Hospice of Huntington. As a Palliative Care Nurse, you will provide high-quality patient care and support to individuals and their families in our community.
Key Responsibilities- Provide full range of nursing care to patients and families in the Palliative Care Program under the supervision of an RN.
- Assist with personal care, such as bathing, and provide emotional support to patients and their loved ones.
- Monitor patients' medical conditions and report any changes to attending physicians and RNs.
- Work collaboratively with an interdisciplinary team to provide comprehensive care and support to patients and their families.
- Implement pain management and symptom management plans to ensure patients' comfort and well-being.
- Respond to the physical, mental, social, and emotional needs of terminally ill patients and their families.
- License to practice by the West Virginia Board of Examiners for Licensed Practical Nurses.
- Must also possess license in Ohio or have an application in progress.
- Minimum of 1-year patient care experience; hospice or home health experience preferred.
- Proof of liability insurance.
- Current driver's license, automobile insurance, and reliable transportation.
- Philosophy compatible with the hospice concept.
- Demonstrates flexibility in scheduling.
- Possesses the ability to travel to all of Hospice of Huntington's service area.
