Home Health Nurse Job Description

Giving Home Health Care Careers is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team in Gallina, NM.

Job Summary

We are looking for a dedicated and caring professional to provide patient care in accordance with a physician-established care plan for 2 patients for 7 hours in total a week.

Responsibilities
  • Execute physician-prescribed plans of care.
  • Complete compliant documentation of the care provided in the system of record.
  • Manage and administer prescribed medication, treatment, and therapies.
  • Conduct patient assessments, coordination of care.
  • Perform various activities associated with daily living.
  • Monitor vitals and GI intake and output.
  • Assess skin integrity and administer wound care.
  • Monitor for changes in the patient's condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
  • Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
  • Provide education and training to the patient, their family, and/or the Home Health Aide.
  • Work with patients to provide a comfortable environment.
  • Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
  • Perform other personal care services as necessary to meet the patient's needs.
Requirements
  • 1+ years of experience as an RN/LPN in a clinical or home health setting.
  • Ability to assist with two-person assist.
