Job Summary

This is a 13-week contract opportunity with the possibility of extension. As a Home Health RN Contractor, you will provide high-quality patient care and support to individuals in their homes, promoting independence and well-being.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessments, treatments, and education.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Conduct regular patient evaluations to assess progress and adjust care plans as needed.
  • Document patient information and care activities in a timely and accurate manner.
Requirements
  • Active Professional licensure within the state of Virginia.
  • At least one year of recent experience in Home Health nursing.
  • BLS/ACLS Certification from the American Heart Association.
