The Palliative Care Counselor provides critical psycho-social support to patients and their families affected by chronic and/or life-limiting conditions. This role involves conducting assessments of patient and family mental health needs, providing consultation to healthcare professionals, and offering counseling services to patients and their loved ones.
Key Responsibilities:
- Conduct thorough assessments of patient and family mental health needs
- Provide counseling services to patients and their families
- Offer consultation to healthcare professionals on the impact of psycho-social factors on patients' illness
- Collaborate with the palliative care consult team to provide comprehensive care
Requirements:
- Masters degree in a relevant field
- Licenses/Certifications: LPC-Licensed Professional Counselor or LSW - Licensed Social Worker
- 3 years related work experience in social work or counseling
- Excellent interpersonal and communication skills
Preferred Qualifications:
- LCSW - Licensed Clinical Social Worker or LCPC - Licensed Clinical Professional Counselor
