Join Our Team as a Visit Licensed Practical Nurse (LPN)

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in Crivitz and Wausaukee for day visits. As a part of Almost Family, a leading home healthcare provider, we prioritize building meaningful relationships with our patients, families, and communities.

Key Responsibilities:

  • Provide high-quality clinical services to patients in their homes, following the plan of care and state laws governing nursing practice.
  • Document patient visits thoroughly and accurately in the electronic medical record, adhering to regulatory and payer requirements.
  • Participate in the agency's care coordination process, communicating effectively with patients, families, physicians, and other healthcare professionals.
  • Ensure patient confidentiality and maintain a safe and healthy work environment.

Requirements:

  • Current LPN licensure in the state of practice.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
  • Current CPR certification.

Preferred Qualifications:

  • Previous home care experience.
