Job Summary:

We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our healthcare team, you will be responsible for providing high-quality care and services to our residents, ensuring their safety and well-being.

Responsibilities:
  • Medication Administration:
    • Administer medications and treatments in a safe and organized manner.
    • Review and document medication administration records.
    • Collaborate with the Resident Care Director and Wellness Nurse to ensure accurate medication administration.
    • Ensure medications are passed according to schedule and document any refusals or missed doses.
  • Resident Care:
    • Provide resident care while promoting dignity and physical safety.
    • Conduct Service and Health Updates as directed by the Resident Care Director.
    • Participate in the development and implementation of Individualized Service Plans (ISPs).
  • Risk Management and General Safety:
    • Partner with the community team to ensure compliance with OSHA and other regulatory requirements.
    • Practice safety procedures, including Personal Protective Equipment (PPE) and fire extinguisher training.
    • Report any accidents or incidents immediately.
Qualifications:
  • LPN, LVN, or state/province specific licensed nurse credential.
  • CPR and First Aid certification.
  • Previous experience working with seniors preferred.
  • Ability to make choices and decisions in the best interest of residents.
  • Ability to react calmly in difficult situations.
