Role Overview

As a Palliative Care RN Case Manager, you will be responsible for planning, organizing, and overseeing hospice care. You will collaborate closely with an interdisciplinary team to tailor services that meet the unique needs of patients and their families within their own homes and communities.

Key Responsibilities
  • Conduct ongoing, thorough assessments of patients and families to identify hospice needs.
  • Initiate and update the care plan as the patient's condition and requirements evolve.
  • Administer prescribed medications and treatments in accordance with the physician's care plan.
  • Maintain accurate and relevant clinical documentation regarding the patient's condition and care provided.
  • Coordinate with community health professionals to ensure a cohesive care plan.
  • Educate patients and family members on self-care techniques as appropriate.
  • Participate in on-call duties as outlined in the on-call policy.
