We are seeking a dedicated and compassionate Hospice Nurse to join our team in West Michigan. As a key member of our Interim HealthCare Home Health and Hospice team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary

This full-time position offers a competitive salary of $80,000 - $100,000 per year, based on experience, plus benefits and bonuses. As a Hospice Nurse, you will be responsible for delivering comprehensive care to patients in their homes, focusing on symptom management, pain control, and quality of life improvement.

Responsibilities
  • Conduct ongoing assessments of patients' physical and psycho-social status, developing personalized plans of care in collaboration with the Interdisciplinary Group (IDG) and attending physicians.
  • Provide education and interventions to support effective management of patients' pain and symptoms, assessing their response to treatment and making necessary revisions to the IDG plan of care.
  • Ensure equipment, supplies, medications, and other essential items are available for patients' needs.
  • Maintain accurate and timely documentation of patient services provided, participating regularly in IDG meetings.
Requirements
  • RN with active, unrestricted license in the State of Michigan.
  • At least one year of relevant experience as an RN, with preference given to candidates with home health or hospice experience.
  • Current CPR Certification and reliable transportation with a valid state driver's license and current car insurance.
