Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Upstate Caring Partners. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals of all abilities and their families.

Key Responsibilities:
  • Implement individual program plans to promote independence and self-care.
  • Attend to personal, self-care, and other program participant needs.
  • Provide support and consultation to program participants and initiate peer support.
  • Oversee the safety and well-being of program participants.
  • Ensure compliance with government and agency regulations and operating standards.
Requirements:
  • A.A.S. Degree or High School plus Specialized Trade Certificate.
  • Valid NYS Licensure as a Licensed Practical Nurse and current NYS Registration Certificate.
  • Strong interpersonal and communication skills.
  • Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel).
  • Ability to travel and possess a valid NYS Driver's License.
