Job Title: Physical Therapist
Job Summary:
We are seeking a highly skilled and compassionate Physical Therapist to join our team at Choice Physical Therapy & Wellness, Inc. As a Physical Therapist, you will play a vital role in helping our patients recover their independence and mobility. Responsibilities:
- Initial and Ongoing Evaluations: Perform thorough initial and ongoing physical therapy evaluations according to APTA standards of practice and state standards.
- Plan of Treatment: Develop a personalized Plan of Treatment for each patient based on initial and ongoing evaluations, including short and long-term goals, collaboration with other staff members, and required patient and caregiver involvement.
- Implementation and Progress Monitoring: Implement the Plan of Treatment on a consistent schedule and adjust as needed based on patient progress.
- Documentation: Document patient progress according to approved medical documentation and facility documentation procedures.
- Communication: Report patient progress to patients, patient caregivers and loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care of patients.
- Discharge Planning: Develop a plan for patients following completion of their Plan of Treatment, including appropriate in-home care services.
- Professional Development: Maintain professional and technical knowledge through attending educational workshops, reviewing professional publications, etc.
- Team Collaboration: Assist other facility staff members as needed to provide care and create a clean, orderly, and effective environment for rehabilitation and physical therapy.
Qualifications:
- Proven Track Record: Proven record of matching patient needs to treatment plans and implementing these plans safely and effectively.
- Experience and Knowledge: Experience in Mechanical Diagnosis and Treatment is preferred but not required. Knowledge of responsible use of facility equipment, best practices for equipment use, and preventative maintenance procedures.
- Assistive Equipment and Follow-up Programs: Experience with recommending assistive equipment to patients and outpatient or home health follow-up programs.
- Age Groups and Injuries: Experience working with a wide range of patient age groups and injuries.
- Empathy and Motivation: Strong sense of empathy, compassionate outlook on PT, and ability to motivate patients into pushing their limits in a responsible and safe way.
- Education and Licensure: BS, Master's Degree in Physical Therapy, or Doctorate Degree in Physical Therapy. Current or eligible for VA Physical Therapy license for the state of operation.
