At Faith Hospice, we employ a team of highly skilled professionals, including nurses, medical social workers, chaplains, home health aides, and volunteers. Together, we deliver comprehensive care that encompasses medical, emotional, and bereavement support.
Key Responsibilities:
1. Assess patient conditions and comfort levels, communicating any changes to the healthcare team and family members.
2. Develop and adjust personalized care plans to manage discomfort and enhance symptom relief.
3. Administer medications and treatments while collaborating with the Interdisciplinary Group for effective care planning.
4. Provide specialized hospice training to staff and caregivers to ensure optimal patient care.
5. Educate patients and families on disease processes while offering emotional support.
6. Supervise LPNs and paraprofessionals, maintaining clear communication with the healthcare team.
7. Foster trusting relationships with patients and families, ensuring timely documentation of care provided.
Qualifications:
- RN licensure in Michigan
- Minimum of 2 years of experience in med/surg, acute care, or skilled nursing
- Preferred experience in hospice or hospital settings
- A strong passion for hospice nursing
