Job Title: Triage Nurse LPN

Job Summary:

We are seeking a highly skilled and compassionate Triage Nurse LPN to join our team at West Hawaii Community Health Center. As a Triage Nurse LPN, you will play a critical role in ensuring the delivery of quality patient care in a responsive and caring environment.

Key Responsibilities:

  • Work with the Nurse Manager, Director of Nursing, and Medical Director to ensure an integrated healthcare delivery system that is efficient, effective, and timely.
  • Plan and oversee health service delivery, make assignments, supervise, and evaluate the quality of care delivered.
  • Screen and respond to patient phone calls and walk-in patients, directing them to Primary Care Providers as needed.
  • Accurately document patient encounters in the EHR system and maintain confidentiality of patient information.
  • Collaborate with the Nurse Manager to ensure the safety and functionality of clinical systems by coordinating maintenance and repair of medical and nursing equipment.
  • Perform clinical case management as needed, including contacting, teaching, and following protocol for patients with abnormal laboratory or imaging results.

Requirements:

  • High School graduate or GED certificate and completion of an accredited LPN program.
  • Current LPN Nursing license for the State of Hawai'i.
  • Current BLS certification.
  • Knowledge of leadership and management principles, clinical management techniques, and state and federal laws and regulations.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups of people.

Working Conditions:

The work environment is usually moderate, with occasional exposure to patients' conditions and unpleasant sights, smells, and contagious diseases. Normal office worker safety precautions and practices are required.
