Key Responsibilities:

  • Assist in the creation, evaluation, and modification of care plans.
  • Deliver nursing services in alignment with the physician's treatment plan and the individualized care plan as authorized by the client's insurance provider.
  • Monitor, document, and communicate any reactions to treatments and changes in the client's health status to the relevant personnel and/or physician.
  • Provide ongoing guidance and oversight to clients, families, or other healthcare team members regarding the treatment plan.
  • Engage in educational programs, quality assessment initiatives, nursing meetings, and other required staff gatherings.

Qualifications:

  • Registered Nurse License in Texas
  • Preferred experience in Home Health Care
  • Excellent organizational and self-management abilities
  • Compassionate customer service skills
  • Valid Driver's License
