Hospice Registered Nurse

We are seeking a compassionate and skilled Hospice Registered Nurse to join our team at Knute Nelson. As a Hospice RN Case Manager, you will provide high-quality patient care and support to individuals and their families in the Wadena/Baxter area.

• Provide Case Management for Hospice Patients
  • Assess patient needs and develop individualized plans of care
  • Collaborate with the interdisciplinary team to ensure comprehensive care
  • Provide education and support to patients and families
  • Supervise Hospice Aides and ensure quality care
• Active Member of the IDG
  • Communicate effectively with the IDG and other healthcare professionals
  • Participate in care conferences and ensure timely updates
  • Refer patients to ancillary services as needed
• Accountability/Leadership
  • Practice accountability and take initiative to achieve company goals
  • Lead by example and foster a positive team culture
  • Identify and address areas for improvement
• Other Duties as Assigned

• Current Registered Nurse licensure in the state of Minnesota
• Current BCLS certification
• Valid driver's license and reliable transportation
• Adequate computer skills and ability to communicate effectively
• Ability to pass a state required background study
