MDS Nurse Coordinator

We are a non-profit 501(c)(3) health and human service organization, providing senior housing options rooted in respect and community for over 100 years.

We are seeking an experienced MDS Nurse Coordinator to join our team. As a critical member of the healthcare team, you will be responsible for all aspects of the MDS process, including care planning and meeting with families regarding the care of their loved one.

This role is perfect for those who are detail-oriented, have the ability to lead and deliver high-quality services in a person-centered environment, and possess excellent communication skills.

• Care Planning: Coordinate the development, completion, and transmission of assessments in accordance with regulatory requirements.
• Reimbursement Management: Review and manage reimbursement related to MDS processes.
• Care Case Management: Responsible for case managing care plans, including rehabilitative programs.
• Regulatory Compliance: Demonstrate knowledge of clinical standards of practice, regulations, and reimbursement governing long-term care and Medicare participation.

• Graduate of approved nursing program.
• Valid Ohio Registered Nurse (RN) license.
• MDS and Restorative Nursing experience.
• Sufficient level computer skills to perform electronic medical record/submission duties.

$85,000 - $105,000 per year, based on location and experience.
