Job Summary

We are seeking a skilled Registered Nurse to join our Home Health and Hospice team. As a Registered Nurse, you will be responsible for providing high-quality patient care in the comfort of patients' homes or in our inpatient hospice facility. Your expertise will be essential in developing and implementing individualized care plans, educating patients and families, and promoting a positive patient experience.

Key Responsibilities
  • Assess patients' physical and emotional needs, and develop personalized care plans to meet their unique requirements.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure comprehensive patient care.
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and promote patient engagement.
  • Document patient care and services provided, including progress notes, medication reconciliation, and other relevant information.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Current North Carolina RN licensure or compact RN licensure in participating state.
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
  • Two years of med-surgical, critical care, or home health/hospice/infusion experience.
  • Valid driver's license and current automobile insurance coverage.
  • Knowledge of Medicare and Medicaid regulations, including JCAHO and DFS standards, ICD-9 Coding.
  • Ability to work autonomously and independently in varying environments.
Preferred Qualifications
  • Associate Degree or Diploma, Nursing required.
  • Registered Nurses are not required but encouraged to enroll in a DUHS approved BSN program after completing two years of service of their start date.
  • Prior experience in Home Health, Hospice, or Infusion preferred.
  • Prior OASIS and PDGM experience preferred for home health.
