Job Title: Healthcare Associate

We are looking for a skilled Healthcare Associate to join our team at Ascend. In this role, you will play a vital part in monitoring patient symptoms through technology and telehealth visits, ensuring seamless collaboration with our in-house clinicians.

The ideal candidate will have exceptional attention to detail, allowing them to accurately monitor and document symptoms, health data, and current medical conditions.

Your responsibilities will include:

  • Coordinating with the clinical team to develop follow-up plans for patients being monitored between visits using wireless monitoring devices and mobile apps.
  • Maintaining accurate and up-to-date patient charts, including diagnoses and current conditions.
  • Engaging in regular communication with patients regarding their concerns, current conditions, and treatment responses.
  • Monitoring wireless device and app data for changes in patient responses and health outcomes.
  • Escalating critical issues to clinicians related to patient health.
  • Documenting all patient interactions in our company portal.

To excel in this role, you should have:

  • At least 2 years of experience in primary care, family practice, geriatrics, and disease management.
  • A valid LVN license that is current and in good standing.
  • An associate's or bachelor's degree, preferably with relevant coursework approved by the California Nursing Board.
  • Strong knowledge of Medicare, chronic condition management, remote patient monitoring, and quality metrics.
  • Excellent communication and interpersonal skills, with the ability to effectively interact with patients and clinicians.
  • Proficiency in MS Office, Google, and EMR systems.
  • Ability to multitask, prioritize tasks, and maintain a high level of organization.
  • Quick thinking, problem-solving, and conflict resolution skills.
  • Familiarity with preventative physical exams, aligned with HCC and Annual Wellness Visit guidelines.
  • Desirable background in customer service or sales and marketing.
