Key Responsibilities:
- Maintain effective communication with patients, families, and healthcare team members.
- Provide person-centered care, adhering to best practices and regulatory requirements.
- Assess patient needs, develop individualized care plans, and coordinate care with other healthcare professionals.
- Administer medications, perform medical treatments, and evaluate their effectiveness.
- Supervise and develop certified nursing assistants and medication administration assistants.
- Investigate incidents, participate in quality assurance and process improvement initiatives, and maintain accurate records.
- Collaborate with interdisciplinary teams to achieve quality outcomes and services for patients.
Requirements:
- Licensed Registered Nurse in the State of Wisconsin.
- Active CPR certification.
- Knowledge and/or experience in geriatrics.
- Supervisory experience preferred.
- Knowledge and/or experience with state and federal regulations.
- Commitment to quality outcomes and services for all individuals.
- Ability to relate well to all individuals.
- Ability to maintain and protect the confidentiality of information.
