About the Role:

We are seeking a highly motivated and compassionate Perinatal Services Case Manager to join our team at Community of Hope. As a key member of our staff, you will play a vital role in ensuring access to supportive and educational services for prenatal and postnatal patients, with a focus on participating in prenatal and postpartum healthcare.

The successful candidate will be responsible for maintaining a case load of approximately 30 participants, providing care coordination services to ensure access to perinatal health and early child health services. This includes conducting thorough assessments, following up and referring patients appropriately based on risk, and attempting to complete monthly follow-up on all clients.

You will also provide health education to clients in various settings, with an emphasis on increasing understanding of healthy pregnancy, self-management of care, and the health system. Additionally, you will utilize non-COH data sources to coordinate care for high-risk clients, assist clients in setting short- and long-term goals using motivational interviewing strategies, and develop care plans with designated periods with particular emphasis on risk factors.

This is a full-time position based out of our Family Health & Birth Center in NE, DC, and the Virginia Williams Family Resource Center in NE, DC. The pay rate for this position ranges between $25.00 - $25.96 per hour, and the offer amount is determined by the candidate's education, qualifications, and experience.
