Overview

We are seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health. As a national leader in home-based care, Enhabit is committed to expanding what's possible for patient care in the home.

The ideal candidate will have a minimum of one year of clinical experience and be licensed as a Registered Nurse in the state of employment. They will possess a valid state driver's license and automobile liability insurance.

Responsibilities

The Registered Nurse Case Manager will be responsible for performing visits and completing coordination of client care. They will oversee the frequency of visits for the episode and consult with the physician and office as needed. Accurate OASIS collection, ensuring the medication profile remains current, and timely reporting of lab values to the physician are key responsibilities of this role.

Qualifications
  • Current licensure as a Registered Nurse in the state of employment
  • A minimum of one year of clinical experience
  • Demonstrate knowledge and skill in current nursing practice
  • Possess a valid state driver's license and automobile liability insurance
