About the Role

This part-time occupational therapist position is an exciting opportunity to join our team at Providence Home and Community Care (HCC). As a member of our interdisciplinary team, you will work closely with patients, caregivers, and healthcare providers to deliver high-quality care and support in the community.

Key Responsibilities
  • Conduct occupational therapy evaluations and develop individualized plans of care for patients
  • Implement and coordinate care with the interdisciplinary team, including physicians, nurses, and social workers
  • Communicate effectively with patients, caregivers, and healthcare providers to ensure seamless care transitions
  • Work independently in multiple settings, including the PACE center, patient homes, and congregate living facilities
  • Assume responsibility for participants treated by the occupational therapy assistant (OTA) and occupational therapy aide (OT Aide)
Requirements
  • Bachelor's degree in occupational therapy
  • Oregon occupational therapist license
  • National Provider BLS - American Heart Association certification within 30 days of hire
  • Preferred qualifications: Master's degree in occupational therapy or OTD, 1 year of experience working as an occupational therapist in a geriatric setting
