Key Responsibilities- Provide Quality Patient Care: Deliver evidence-based nursing care to patients in their homes, ensuring their physical and emotional needs are met.
- Collaborate with Healthcare Team: Work closely with physicians, nurse practitioners, and other healthcare professionals to ensure seamless care coordination.
- Conduct Home Visits: Assess patients' needs, provide education, and support them in managing their health conditions.
- Triaging and Communication: Triage patient calls, respond to messages, and communicate effectively with patients, families, and healthcare teams.
- Medication Management: Administer medications, monitor vital signs, and perform routine treatments and procedures as prescribed by physicians.
- Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, care provided, and any concerns or issues.
- Quality Improvement: Participate in quality improvement initiatives, identify areas for improvement, and contribute to the development of best practices.
- Education and Licensure: Graduate of a state-approved or National League of Nursing accredited School of Nursing, RN license or eligible for licensure in Maryland.
- Experience: Two years' experience as a RN, physician office experience preferred.
- Certifications: BLS certification required.
