Speech Language Pathologist Home Health Per Diem

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a key member of our care team, you will work closely with patients, families, and healthcare professionals to provide high-quality speech and language services in the comfort of their own homes.
Responsibilities- Evaluate and treat patients with speech and language disorders, including those related to stroke, traumatic brain injury, and neurodegenerative diseases.
- Develop and implement individualized treatment plans to improve communication skills and quality of life.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between settings.
- Provide education and training to patients and families on communication strategies and techniques.
- Document patient progress and outcomes in accordance with regulatory requirements and company policies.
- Master's degree in Speech-Language Pathology or related field.
- Current licensure and certification in Speech-Language Pathology.
- Minimum of six months experience as a speech therapist or speech-language pathologist.
- Home health experience preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
