Join our interdisciplinary team at Corewell Health in Royal Oak, where we provide value-related activities such as transitions of care, integrated behavioral health, and health-related social needs navigation for all patients. As part of our statewide team, we support complex healthcare journeys across the continuum of care.

Our Mission:

To work collaboratively with healthcare professionals to assist patients and families with complex psychosocial needs, including alternate site coordination, discharge planning, and addressing identified abuse/neglect, substance use disorders, end-of-life decisions, and bereavement support.

We conduct psychosocial assessments with patients and families to identify psychosocial needs or alternative care requirements post-discharge. We communicate effectively with alternate sites and community agencies to address patient needs, complete and document assessments and plans in the electronic medical record, and provide consultation and resources to the healthcare team.

Key Responsibilities:

• Conduct daily group therapy sessions with eligible inpatients and partial program patients
• Provide discharge planning and utilization approvals for post-inpatient care
• Maintain current knowledge of community resources and federal/state regulations relevant to the role

Qualifications and Requirements:

• Master's degree in social work (MSW)
• State of Michigan Social Work License (LMSW) expected within 5 years of employment

Compensation:

The estimated annual salary range for this position is $60,000-$80,000, depending on experience and qualifications.
