Licensed Practical Nurse

This position provides high-quality medical care for residents in a safe and supportive environment. The Licensed Practical Nurse (LPN) will work closely with the healthcare team to develop and implement individualized care plans, ensuring the best possible outcomes for our residents.
Key Responsibilities- Assist with the development and implementation of care plans for residents
- Provide direct care to residents, including administering medications and treatments as prescribed
- Monitor and report changes in resident condition to the healthcare team
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Provide emotional and social support to residents and their families
- Participate in ongoing education and training to stay current with best practices in skilled nursing
- Wisconsin Practical Nurse License
- High School Diploma or equivalent
- Completion of a practical nursing program
- Previous experience as a CNA or LPN preferred
- Ability to work effectively in a fast-paced environment
- Strong communication and interpersonal skills
