Responsibilities- Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the patient's multidisciplinary care team.
- Provide direct patient care, including administering medications, wound care, and other treatments as ordered by the patient's physician.
- Coordinate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care and effective communication.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Document patient information accurately and thoroughly, including progress notes, medication lists, and other relevant data.
- Participate in quality improvement initiatives and contribute to the development of best practices in home health care.
- Current, unencumbered license to practice as a Registered Nurse in the state(s) where you will be working.
- One (1+) year of clinical experience as a Registered Nurse, with a focus on home health care or a related field.
- Current CPR certification.
- Ability to assess patient status and identify requirements relative to age-specific needs.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
