Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Aveanna Healthcare LLC. As a Pediatric Home Healthcare professional, you will provide high-quality patient care in a home setting.

Job Summary:

The successful candidate will be responsible for delivering compassionate and individualized care to pediatric patients in their homes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans in collaboration with the patient's healthcare team
  • Conduct thorough assessments of patients' physical and emotional needs
  • Collaborate with family members and caregivers to ensure seamless care transitions
  • Document patient progress and communicate effectively with the healthcare team
Requirements:
  • Current LPN licensure in the state of employment
  • Minimum 1 year of experience in pediatric home healthcare or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic life support certification
