Nurse Practitioner

The Licensed Practical Nurse (LPN) is a vital member of our healthcare team, providing direct patient care under the supervision of a registered nurse. This role requires a strong commitment to delivering high-quality patient care and working collaboratively with our team to achieve positive patient outcomes.
Key Responsibilities- Provide direct patient care as defined in the state's Nurse Practice Act.
- Implement the plan of care initiated by the registered nurse.
- Document patient care accurately and in a timely manner.
- Collect assessment data and perform observations to report to and collaborate with RNs and physicians.
- Participate in the coordination of home health services and identify needs for other disciplines.
- Use equipment and supplies effectively and efficiently.
- Participate in personal and professional growth and development.
- Perform other duties as assigned by the registered nurse.
- Must have completed a practical nursing program or be licensed in the state where the LPN is located.
- Three years of nursing experience preferred, with community/home health or medical/surgical experience being a plus.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Comply with accepted professional standards and practice.
- Demonstrate good verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possess and maintain current CPR certification.
