Key Responsibilities of the MDS Care Coordinator:

* Engage actively in creating a detailed, personalized care plan for each resident.

* Ensure that the resident care plan is completed, updated, and reviewed in line with RAI guidelines and any changes in resident needs.

* Take charge of the execution of Clinical and Prospective Payment System Minimum Data Set completion schedules.

* Organize and schedule necessary resident assessments and documentation for Medicare, Medicaid, managed care, and Health Maintenance Organization (HMO) reimbursement.

* Oversee all aspects of the MDS assessment process to ensure compliance with federal and state regulations.

* Participate in the pre-admission process to assess potential residents' care needs.

* Contribute to determining residents' care levels upon admission.

* Monitor changes in residents' care levels to ensure timely scheduling of MDA assessments.

* Review all MDS assessments and related documentation to confirm timely completion by signing the appropriate sections.

* Analyze reports on the timely completion of residents' MDS assessments and care plans, taking necessary follow-up actions.

* Evaluate Quality Indicator reports for potential resident issues and ensure this information is reflected in care plans.

* Maintain up-to-date knowledge of state and federal regulations regarding the RAI process, care planning, and the prospective payment system (PPS).

Qualifications for the MDS Care Coordinator:

• RN/LPN License

• Experience with MDS
