Digestive Health Nurse Care Coordinator

About the Role:
The Digestive Health Nurse Care Coordinator plays a vital role in ensuring seamless coordination of patient care for Digestive Health patients. Reporting to Nursing leadership within the Digestive Health Center, this position requires a Registered Nurse with a strong focus on clinical practice, care coordination, education, and quality improvement.
Key Responsibilities:
Provides patient and family education on medical conditions, including signs, symptoms, treatment regimens, side effects, and outcomes expected.
Functions as a key member of the multi-disciplinary team, coordinating all aspects of patient care across the continuum.
Participates in data collection related to clinical and operational efficiencies.
Requirements:
Registered Nurse in the State of Connecticut.
At least 2-3 years of experience in a gastroenterology inpatient or outpatient setting, including experience in coordinating services and care for gastroenterology disorder patients.
