About the Role

We are seeking a highly skilled Occupational Therapist to join our multidisciplinary team at our Skilled Nursing Facility. As a key member of our care team, you will be responsible for providing high-quality occupational therapy services to patients requiring skilled care.

Responsibilities
  • Assess, plan, implement, and evaluate individualized care plans for patients requiring occupational therapy services.
  • Provide occupational therapy as ordered by a physician, in accordance with the designated plan of care.
  • Review and explain physician orders with patients and their families.
  • Coordinate with care team members to provide high-quality services and care to all patients.
  • Record patient treatments and evaluate progress and effectiveness in accomplishing goals.
Requirements
  • Provide occupational therapy care that complies with our organization's best practices, standards, policies, and procedures.
  • Comply with federal and state guidelines, including all licensing requirements.
