Job Summary
We are seeking a highly skilled Physical Therapist to join our team at Concord HOMEChoice, a reputable home health care agency in Rhode Island. As a Physical Therapist - Home Care Specialist, you will be responsible for performing admissions and visits for seniors in a home care setting.
About the Role
This is a full-time position that requires the ability to work independently and as part of an interdisciplinary team. You must have excellent assessment skills and be willing to travel throughout RI.
Requirements
- Masters degree in Physical Therapy
- At least 2 years of experience as a PT, preferably in home care
- Current OASIS/Medicare certification and electronic health records experience preferred
Compensation Package
The salary for this position is $$90,000 per year, which includes good pay rate, mileage reimbursement, 401K, vacation time, and health/dental/vision insurance. In addition, we offer up to $5,000 signing bonus.
