Advanced Practice Clinician

Our medical group provides home-based medical care to patients, allowing our clinical teams to spend quality time caring for a smaller number of patients and giving each patient the space, respect, compassion, and care they deserve.
Key Responsibilities:- Collaborate with interdisciplinary teams to develop and carry out patients' goals of care in all phases of the patient journey.
- Establish and maintain effective relationships with members and their legal representatives, facility staff, PCPs, and other collaborating physicians/providers.
- Perform initial comprehensive assessments, develop member management care plans, and update them as needed.
- Prescribe appropriate diagnostics and interventions to avoid unnecessary transfers or acute admissions.
- Consult with hospital physicians following notification of member transfer.
- Educate members and/or their legal representatives in disease processes affecting members and ways to manage them effectively.
- Completion of Nurse Practitioner or Physician Assistant accredited program.
- Current state RN license if Nurse Practitioner.
- Current NP or PA license.
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Proven knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- 2+ years of clinical experience.
- Knowledge of CPT, ICD-9, and HCPC codes.
- Knowledge of clinical standards of care.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
**Note:** Employees must be in an active regular status and remain in the role for a minimum of 12 months from the date of hire/rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
