Job Title: Physical Therapist - Home Health Care Specialist

Pinnacle Home Care Inc. is seeking a skilled Physical Therapist to join our team of dedicated healthcare professionals. As a Physical Therapist - Home Health Care Specialist, you will be responsible for providing high-quality physical therapy services to patients in their own homes.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs and develop personalized treatment plans.
  • Deliver physical therapy services to patients in their homes, adapting treatment plans to suit individual needs and environments.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination.
  • Teach patients and their families how to perform exercises and use adaptive devices to promote independence and mobility.
  • Document patient progress and outcomes, ensuring accurate and timely reporting.
Requirements:
  • Doctorate or better in Physical Therapy from an accredited program.
  • Current licensure or registration as a Physical Therapist in the state of practice.
  • At least one year of experience in an acute or sub-acute setting, with a strong focus on home health care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
