Job Title: Master's Level Social Work Professional

About the Opportunity:

This is an exciting opportunity to join our team as a Master's Level Social Work Professional. You will work closely with patients and their families to provide supportive advocacy and help overcome barriers to progression-of-care.

Key Responsibilities:

  • Develop and implement patient-centered care plans in collaboration with healthcare professionals.
  • Conduct thorough assessments to identify patient needs and develop individualized plans to address these needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate and detailed records of patient interactions and progress.

Requirements:

  • Master's Degree in Social Work (MSW)
  • Previous experience in mental health preferred
  • Good communication skills and ability to communicate effectively in written format and/or oral presentations
  • Strong analytical & problem-solving skills; semi-independent in decision making
  • Ability to maintain organization in a changing environment
  • Exhibit initiative, responsibility & flexibility
  • Ability to initiate and understand research related to projects
  • Learn and appropriately utilize all required computer applications
  • Ability to write effective documentation for processes and procedures
  • Ability to make routine decisions in accordance with departmental policies and procedures
  • Appropriate assessment skills
