Job SummaryWe are looking for a dedicated and experienced RN to work on a per diem basis, providing nursing care to adult and geriatric clients in their homes. As a Per Diem RN, you will have the opportunity to work with a variety of clients, each with unique needs and circumstances.
Responsibilities- Provide high-quality nursing care to clients in their homes, following a designated care plan and adhering to established protocols and procedures.
- Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing effective care plans to address these needs.
- Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
- Monitor clients' conditions, reporting any changes or concerns to the Clinical or Client Services Manager.
- Document all interactions with clients, including assessments, interventions, and evaluations, in a timely and accurate manner.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care for clients.
- A current license as a Registered Nurse in Vermont.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred, but not required.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and ability to communicate effectively with clients, families, and healthcare professionals.
- Solid computer skills and prior experience with electronic medical records (EMR) is preferred.
