Hospice Care Manager Position

Job Summary:
We are seeking a skilled and compassionate Hospice Care Manager to join our team at St Croix Hospice. In this role, you will be responsible for planning, organizing, and directing hospice care for patients in their homes and communities.
About the Role:
- This is a full-time position with a competitive salary of $$85,000 - $110,000 per year, depending on experience.
- You will work closely with interdisciplinary teams to coordinate patient care and provide emotional support to patients and their families.
- Your primary responsibility will be to assess patient needs, develop individualized care plans, and ensure seamless delivery of services.
- You will also supervise hospice aides, communicate effectively with healthcare providers, and participate in quality improvement initiatives.
Requirements:
- A current license to practice as a Registered Nurse (RN) in your state.
- At least one to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care preferred.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
