Job Title: LPN Community Nurse

Lifesteps, a non-profit human services organization, is seeking a Full-Time LPN Community Nurse for the Washington County area, based in Eighty Four, PA.

Job Summary

As a key role in healthcare provision for people with intellectual disability, the LPN Community Nurse will support person-centered care and healthy outcomes. The Nurse will be a liaison and advocate for IDD adult individuals in Lifesteps Community Homes, supporting and educating direct care staff and communicating health changes to supervisors, directors, and medical professionals.

Responsibilities
  • Assist individuals with medical appointments and obtain medical records and instructions from doctors
  • Verify medical recommendations are being implemented and staff is being trained in the care of individuals' needs
  • Document client diagnosis and medications, and staff training
  • Travel to assigned areas to check in on individuals' needs, using a personal vehicle
Requirements
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with an active PA license
  • One (1) year experience working in a nursing field
  • Demonstrated ability to complete basic computer tasks
  • First Aid/CPR certification or the ability to complete course within 6 months of hire
  • Current driver's license and the ability to travel for business using a personal vehicle
Preferred Qualifications
  • Experience with Intellectual Developmental Disability and/or special education
  • Experience in community nursing, home care, and/or school nursing
