Responsibilities- Conduct regular wellness visits for all residents to identify and address any health concerns.
- Collaborate with the Resident Care Director to develop and implement individualized care plans.
- Monitor and document resident vital signs, including weights and blood pressure.
- Provide clinical support and assistance to community team members as needed.
- Participate in quality assurance meetings and contribute to the development of quality care indicators and outcomes.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in a healthcare setting, preferably in a long-term care environment.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
