Key Responsibilities:
- Manage the MDS (Minimum Data Set) process, including assessments, data entry, and submissions to ensure adherence to regulations.
- Collaborate with a multidisciplinary team to create personalized care plans tailored to residents' needs.
- Review and monitor the accuracy of resident assessments and care plans, making necessary adjustments.
- Participate in care plan meetings, offering insights and recommendations based on assessments.
- Maintain accurate documentation and records for all MDS-related activities.
- Provide training and guidance to staff regarding MDS processes and compliance.
Qualifications:
- Valid California LVN license.
- Previous experience in an MDS Coordinator role or similar position within a skilled nursing environment.
- In-depth knowledge of MDS 3.0, RAI processes, and CMS guidelines.
- Strong communication, organizational, and analytical skills.
- Ability to work independently as well as collaboratively within a team.
