Job Summary:

The Case Management Coordinator plays a vital role in facilitating post-acute care arrangements for patients. This individual responds to service requests from Case Management staff, identifies obstacles, and works collaboratively to resolve issues. The Coordinator documents actions, interactions, and arrangements in the medical record and serves as a resource for information about post-acute care services.

Key Responsibilities:
  • Facilitate Skilled Nursing Facility, hospice, acute rehab, LTAC, home health, and home medical equipment referrals
  • Disseminate IMM letters to patients
  • Schedule patient appointments and contact pharmacy for medication pricing
  • Complete payer forms, transfer packets, and non-clinical sections of wound vac forms
  • Arrange patient transportation and verify insurance coverage

Requirements:
  • Associate's degree preferred
  • Minimum one to three years healthcare experience
  • Proficient in computer systems and software

Work Requirements:
  • Day shift
  • No on-call requirements
  • Rotating weekends
  • No travel required
