This role requires:

  • Coordination of care and services for assigned clients, including therapy
  • Observation, recording, and reporting signs and symptoms, reactions to treatments and medications, and any changes in the client's physical or emotional condition in a timely manner
  • Teaching clients and family members about proper care and effective management of medical conditions
  • Arrangement of counseling services for the client and family as appropriate
  • Accurate completion of OASIS documentation
  • Discharge planning and preparation of discharge summaries and instructions
Key Responsibilities

Your primary responsibilities will include:

  • Developing and implementing individualized care plans
  • Providing skilled nursing services to patients in their own homes
  • Collaborating with multidisciplinary teams to coordinate patient care
  • Communicating effectively with patients' physicians to ensure seamless care transitions
  • Ensuring accurate and timely completion of OASIS documentation

Earn an estimated $25.50/hour (LPN) or $30.78/hour (RN), based on location and experience.
