About the Role

We are seeking a skilled Care Coordinator to join our team at LifeBridge Health. As a Care Coordinator, you will play a vital role in promoting the health and well-being of our patients by providing personalized support and connecting them with community resources.

Key Responsibilities
  • Assess and address patient needs: Conduct thorough assessments of patients' physical and social needs, and develop personalized plans to address these needs.
  • Link patients with community resources: Connect patients with local resources, such as prescription assistance programs, social services, and healthcare providers.
  • Coordinate care: Work with patients, caregivers, and healthcare providers to ensure timely access to services and appointments.
  • Monitor and evaluate patient progress: Regularly review patients' progress and make adjustments to care plans as needed.
  • Document patient information: Maintain accurate and up-to-date records of patient interactions and care plans.
Requirements
  • Education and experience: A licensed practice nurse, certified medical assistant, or trained patient care assistant with 2-3 years of acute care and/or ambulatory practice experience.
  • Communication and organizational skills: Excellent verbal and written communication skills, as well as strong organizational and time management skills.
  • Electronic medical records: Competency in electronic medical records is desirable.
  • Language proficiency: Bi-lingual proficiency is preferable, particularly in market-specific languages.
