About the Role

Amedisys, Inc. is seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Hospice On-Call RN. As a key member of our hospice care team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.

Responsibilities
  • Perform initial nursing assessments on new admissions to ensure comprehensive care planning.
  • Develop, coordinate, and revise individualized patient care plans in collaboration with the care team.
  • Regularly communicate patient progress to the clinical manager, physician, and care team to ensure seamless care coordination.
  • Document patient care progress accurately and in a timely manner, adhering to care center policies.
  • Provide education to families on patient condition, needs, and care to ensure informed decision-making.
  • Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
  • Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Requirements
  • Current RN license, specific to the state(s) assigned to work.
  • One+ year of nursing experience required.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
