Responsibilities- Perform initial nursing assessments on new admissions to ensure comprehensive care planning.
- Develop, coordinate, and revise individualized patient care plans in collaboration with the care team.
- Regularly communicate patient progress to the clinical manager, physician, and care team to ensure seamless care coordination.
- Document patient care progress accurately and in a timely manner, adhering to care center policies.
- Provide education to families on patient condition, needs, and care to ensure informed decision-making.
- Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Current RN license, specific to the state(s) assigned to work.
- One+ year of nursing experience required.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
