Job Description:
We are seeking a skilled and compassionate Certified Nursing Assistant to join our team as a Home Health Assistant. In this role, you will provide high-quality, person-centered care to our clients in their homes, supporting them with daily activities, monitoring their well-being, and ensuring they receive the highest standard of care to promote comfort, safety, and independence.
Responsibilities:
- Personal Care: Assist clients with daily activities, including bathing, grooming, dressing, and toileting, while maintaining their dignity and independence.
- Mobility Support: Aid clients with transferring, positioning, and ambulation, ensuring safe practices.
- Vital Signs Monitoring: Regularly monitor and document clients' vital signs, including temperature, pulse, respiration, and blood pressure, as directed.
- Medication Reminders: Prompt clients to take prescribed medications according to schedule.
- Meal Preparation: Prepare and serve meals according to clients' dietary needs and preferences, promoting nutrition and hydration.
- Household Assistance: Perform light housekeeping duties, such as cleaning, laundry, and organizing to maintain a clean and safe environment.
- Companionship: Offer emotional support, companionship, and engage clients in meaningful activities to enhance their quality of life.
- Health Observations: Report changes in clients' conditions or behavior to the supervising nurse or care coordinator.
- Accurate Documentation: Maintain accurate, timely, and detailed records of care provided and client observations.
Requirements:
- A minimum of 18 years of age.
- A clear background check as per state regulations.
- A valid driver's license and reliable transportation.
- Current CPR certification is preferred.
- Negative TB test or chest x-ray, updated annually.
