Job Summary

We are seeking a dedicated clinical professional to join our team as an Oncology Pharmacist. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring the safe and effective use of medications in our oncology department.

Responsibilities
  • Rotate shifts and areas with a primary focus in oncology and CAR-T program patients
  • Prepare and dispense drugs in all inpatient settings
  • Maintain and review patient medication profiles
  • Review and verify physician medication orders and patient medical records
  • Interact with healthcare professionals and providers to promote safe medication use and prevent inappropriate or misuse of medications
  • Provide clinical pharmacy services, including pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, and pharmacist interventions
  • Verify new admission medication orders, medication history taking, and reconciliation of high-risk patients upon admission
  • Round with MD/RNs beginning on admission to provide effective medication therapy during the patient's stay and early planning for discharge
Requirements
  • PharmD degree and current CA Pharmacy license
  • ASHP-accredited residency (preferred)
  • Completion of PGY1 Pharmacy Residency or equivalent 3-5 years experience in inpatient hospital pharmacy setting and/or in decentralized patient care pharmacy setting
  • Prefer clinical background/residency in oncology and/or Board Certification in Oncology
  • Exceptional communication, interpersonal, organizational, and follow-through skills
  • Knowledge of and ability to perform age-specific and disease-specific pharmacokinetic calculations
  • Ability to compound intravenous admixtures and other sterile products
