Job Title: Pharmacist

InfuCare Rx Inc, a leading home infusion provider, is seeking a skilled Pharmacist to join our team. As a Pharmacist, you will play a vital role in ensuring the safe and effective delivery of home infusion therapies to our patients.

Job Summary:

The successful candidate will be responsible for coordinating and facilitating new referral/admissions, receiving and verifying patient orders, and conducting initial and ongoing assessments of patient, drug, and disease information. You will also assist physicians and other healthcare professionals in planning and implementing patient care, and monitor the outcomes of drug therapy.

Key Responsibilities:
  • Coordinate and facilitate new referral/admissions through collaboration with nursing, customer service, and distribution.
  • Receive, review, and verify all new patient orders and documents.
  • Assist physicians and other healthcare professionals in planning and implementing patient care.
  • Conduct initial and ongoing assessments of patient, drug, and disease information.
  • Monitor the outcomes of drug therapy by collecting, evaluating, and interpreting laboratory data, clinical data, and other pertinent patient information.
  • Communicate the results of therapeutic drug monitoring activities to physicians, nursing, and other appropriate persons.
  • Program infusion control devices as indicated.
  • Provide consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drug and their use.
  • Maintain thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians, and other healthcare professionals.
  • Participate in the preparation of progress reports for the referring physician on a regular, periodic basis.
  • Supervise, coordinate, and/or participate in the preparation of sterile admixtures, dispense all pharmaceutical and related supplies and equipment in accordance with federal, state, and local laws and standards of practice which govern the profession of pharmacy.
  • Process and check all medications, supplies, and equipment prior to dispensing from the pharmacy.
  • Maintain pharmacy records in accordance with applicable laws and regulations.
  • Maintain clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars, and conferences.
  • Participate in on-call rotation if applicable.
  • Perform marketing support functions as requested.
  • Participate in orientation/training programs for new personnel and current staff.
  • Participate in departmental QA activities.
  • Attend designated department meetings as scheduled.
  • Perform other tasks or special projects as requested by management.
Requirements:
  • A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited college of Pharmacy.
  • Currently Licensed as a registered pharmacist in the state of employment.
  • Possess a minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy.
  • Possess experience and strong knowledge base in intravenous admixture services and intravenous therapy.
  • Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionals.
  • Can work effectively with others to provide safe and effective drug therapy.
  • Can communicate, both verbally and written, with patients, clients, management, and other employees.
  • Can problem solve and supervise technical and other support staff.
  • Must be flexible with work schedule.
  • Possess good organizational and time-management skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Dependable in attendance and job performance.
Physical Demands:

Required to stand, walk, sit, talk, and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close, distance and peripheral vision, and the ability to adjust focus, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
