North Mississippi Health Services is seeking a skilled Per Diem Case Manager to join our team. As a key member of our healthcare team, you will be responsible for conducting patient assessments, implementing interventions, and coordinating transition of care to post-acute services.
Key Responsibilities- Conduct thorough patient assessments to determine needs and barriers to disease management or recovery.
- Implement appropriate interventions to address identified patient needs and maintain regulatory requirements.
- Monitor the effectiveness of patient care plans and intervene when necessary to address potential progression delays.
- Arrange post-acute services and procure durable medical equipment as needed.
- Coordinate transition of care to ensure seamless patient care.
- Associate's or Bachelor's degree in Nursing, Social Work, or Counseling.
- 3 years of experience in nursing, social work, or counseling.
- RN licensure and/or compact state licensure.
- Board Certified Case Manager certification within 1 year of accepting position.
- Cardiopulmonary Resuscitation (CPR) certification.
- Master's degree in Nursing, Social Work, or Counseling.
- Experience in case management or a related field.
